Imaginário coletivo de profissionais do CAPS AD sobre o cuidado a familiares de pessoas que fazem uso dependente de álcool

Abstract

Os profissionais do CAPS AD vivenciam inúmeros desafios no trabalho, particularmente quando há carência de recursos, sobrecarga de tarefas e insegurança sobre o papel profissional. Dentre os desafios, destaca-se o cuidado com a família do usuário a qual, frequentemente, demanda da equipe um conjunto de intervenções, seja para o parente que usa a substância, seja para as próprias necessidades afetivo-emocionais. Considerando o atual cenário brasileiro em que o uso do álcool se sobressai ao das demais substâncias psicoativas, o objetivo do presente estudo é compreender o imaginário coletivo de profissionais do CAPS AD sobre o cuidado a familiares de pessoas que fazem uso dependente de álcool. Para a concretização dessa proposta foram realizadas Entrevistas Transicionais com 13 profissionais da saúde mental que trabalham no CAPS AD. Dado que o presente estudo se insere no panorama das pesquisas qualitativas, cujo foco é investigar os significados humanos, fizemos uso do método psicanalítico para interpretar o imaginário coletivo dos participantes sobre o cuidado aos familiares. Utilizamos o recurso investigativo da Narrativa Interativa (NI), história fictícia com desfecho em aberto que potencializa a associação livre e a expressão emocional dos participantes. Para este estudo foi confeccionada uma NI que aborda o drama dos familiares que buscam auxílio em um serviço de saúde mental. Cada entrevista deu origem a uma Narrativa Transferencial (NT), na qual há uma descrição vivencial do encontro em suastonalidades afetivas. A análise interpretativa do material narrativo reunido pelas NI e NT resultou em dois campos de sentido afetivo emocional que sustentam o imaginário coletivo dos participantes. O primeiro campo é “Apagando Incêndios” que desvela o contexto das crises emocionais dos familiares e a crença dos profissionais a respeito da potencialidade de intervenções breves e focais para atenuar o agudo sofrimento dos familiares durante a experiência da crise. O segundo campo, intitulado “O Peso das Cinzas” revela o sentimento de impotência dos profissionais diante do sofrimento crônico dos familiares. Nesse cenário marcado pelo desamparo, os entrevistados divergem quanto ao lugar da família do usuário no CAPS AD e sobre os fundamentos teóricos que sustentam as intervenções voltadas para esse público. Tecemos reflexões sobre os desafios vividos pelos participantes para elaborar e concretizar o cuidado à família do usuário. Destacamos que na atual conjuntura em que os equipamentos da Reforma Psiquiátrica estão sucateados em termos de recursos humanos e financeiros, os profissionais do CAPS AD necessitam de apoio emocional e formação continuada para o exercício do cuidado aos familiares dos usuários.

Center of Psychosocial Care – Alcohol and Drugs professionals experience countless challenges at work, particularly when there is a shortage of resources, overload of tasks and insecurity about the professional role. Among these challenges, the care for the family of the alcohol dependent person stands out, with the family frequently demanding from the staff a series of interventions, be it for the alcohol dependent relative, or their own emotional needs. Considering the current Brazilian context, in which alcohol use is more prevalent than that of other psychoactive substances, the objective of the present study is to understand the collective imaginary of CAPS AD professionals about the care for the relatives of people who make dependent use of alcohol. In order to achieve this goal, Transitional Interviews were conducted with 13 mental health care professionals who work at a CAPS AD. Given that the present study inserts itself in the panorama of qualitative research, whose focus is to investigate the human meanings, we made use of the psychoanalytical method to interpret the collective imaginary of the participants about the care for the families. We made use of the investigative resource of the Interactive Narrative (IN), an unfinished story that potentializes the free association and the emotional communication of the participants. For this study, an IN that addresses the drama of families that seek help in a mental health care service was designed. Each interview gave origin to a Transferential Narrative (TN), in which there is an experiential description of the encounter and its affective tones. The interpretative analysis of the narrative material made up of the INs and the TNs resulted in two fields of affective-emotional meaning that sustain the imaginary of the participants. The first field is “Putting out fires”, which unravels the context of the emotional crises of the relatives and the belief from the professionals regarding the potentiality of brief and focused interventions to attenuate the acute suffering of the relatives during the crisis experience. The second field, entitled "The Weight of the Ashes", reveals the feeling of impotence from the professional facing the relatives' chronic suffering. In this scenario, marked by helplessness, the interviewees diverge regarding the place of the user's family in the CAPS AD, and about the theoretical foundations which support the interventions aimed at that audience. We reflect upon the challenges faced by the participants to elaborate and concretize the user's family care. We highlight that in the current scenario in which the Psychiatric Reform's equipments are scrapped in terms of human and financial resources, the CAPS AD professionals need emotional support and continuing education to provide care for the families of users.
