Resumo
INTRODUÇÃO: A gestão da aplicação de um pacote de intervenções preventivas em pacientes entubados, como: a higiene oral, manutenção do decúbito do leito elevado, técnica adequada de intubação, cuidados adequados com os equipamento ventilatórios e supervisão na administração da dieta enteral, podem reduzir a incidência de complicações como Pneumonia Associada à Ventilação Mecânica (PAV). Justificativa: Atualizar e desenvolver estratégias para melhorar a adesão da equipe assistencial na aplicação do protocolo em uma UTI-A, promovendo um aprimoramento no cuidado seguro e individualizado aos pacientes. OBJETIVOS: Realizar uma revisão atualizada na literatura sobre o assunto; Levantar a intervenção preventiva com menor adesão na prática e desenvolver uma estratégia para melhorar o índice de efetividade do protocolo de PAV em uma UTI-A. METODOLOGIA: Trata-se de um projeto aplicativo, que tem como eixo a Saúde do Adulto/ Idoso e Gestão em Saúde, terá como cenário de aplicação uma UTI-A, sendo o público alvo a equipe de enfermagem, referência na gestão direta do protocolo. Será desenvolvido uma ferramenta de checklist beira-leito para controle visual da técnica de higiene oral e sua aplicação, a fim de melhorar a adesão do protocolo na prática. RESULTADOS: Os resultados obtidos se deram, a partir da realização de uma reunião com gestores da UTI-A 1 do Hospital PUC-Campinas, com proposta de atualização da pratica assistencial, bem como a disponibilização dos produtos finais anexados nos apêndices desta revisão, para serem replicados para equipe assistencial e realizado a validação do instrumento beira leito e definição de aplicação com a gestão. CONCLUSÃO: O projeto visa implementar medidas preventivas para reduzir a Pneumonia Associada à Ventilação Mecânica (PAV) na UTI, diminuindo a mortalidade e melhorando a gestão hospitalar. Durante a pesquisa, foi identificada uma falha na higiene oral dos pacientes, o que levou à atualização da diretriz sobre o tema. A revisão científica concluiu que a higiene oral deve ser feita pelo menos três vezes ao dia para prevenir a PAV, pois a cavidade oral contaminada pode levar à broncoaspiração de microrganismos. O projeto contribuiu para a revisão das práticas e a promoção de cuidados mais eficazes, superando desafios e gerando impactos positivos no manejo de pacientes em ventilação mecânica.
Abstract INTRODUCTION: Intubated patients should be managed with a package of preventive interventions, such as oral hygiene, maintenance of elevated bed decubitus, adequate intubation technique, adequate care with ventilatory equipment and supervision in the administration of enteral nutrition. OBJECTIVES: Are to conduct an updated review of the literature on the subject, identify preventive interventions that exhibit the lowest adherence in practice, and devise a strategy to enhance the efficacy rate of the VAP protocol in an ICUAI. METHODOLOGY: This is an application project, which has as its axis the Health of the Adult/Elderly and Health Management, will have as an application scenario an AICU, with the target audience being the nursing team, a reference in the direct management of the protocol. A checklist tool next to the bed will be developed for the visual control of oral hygiene techniques and their application, in order to improve adherence to the protocol in practice. RESULTS: Obtained were from the holding of a meeting with ICU-A 1 managers of the PUCCampinas Hospital, with a proposal to update the care practice, as well as the availability of the final products attached in the appendices of this review, to be replicated to the care team and carried out the validation of the bedside instrument and definition of application with management. CONCLUSION: The project aims to implement preventive measures to reduce Ventilator-Associated Pneumonia (VAP) in the ICU, reducing mortality and improving hospital management. During the research, a failure in the oral hygiene of the patients was identified, which led to the update of the guideline on the subject. The scientific review concluded that oral hygiene should be performed at least three times a day to prevent VAP, since a contaminated oral cavity can lead to bronchoaspiration of microorganisms. The project contributed to the review of practices and the promotion of more effective care, overcoming challenges and generating positive impacts on the management of patients with mechanical ventilation.
