Resumo
A obesidade infantil está associada a um estado de inflamação crônica de baixo grau, caracterizado por desregulação da imunidade inata e adaptativa. Alterações estruturais do tecido adiposo, como hipertrofia adipocitária, infiltração de células do sistema imune e fibrose, também estão relacionadas à resistência à insulina, disfunção endotelial e pior perfil metabólico. Na imunidade adaptativa, têm sido descritas alterações em diferentes populações de linfócitos, incluindo desequilíbrios entre células Th17 e Treg, modificações em células B e associações entre essas alterações imunológicas e parâmetros como IMC, HOMA-IR, metabolismo glicídico e perfil lipídico, reforçando o papel da resposta imune adaptativa na fisiopatologia da obesidade pediátrica. Além disso, alterações em células NK e no metabolismo energético de células imunes indicam uma reprogramação imunometabólica precoce associada ao excesso de adiposidade. Em conjunto, as evidências sugerem que a obesidade na infância e adolescência promove ativação persistente do sistema imunológico associada a alterações metabólicas. Compreender melhor essas interações poderá contribuir para o desenvolvimento de novas estratégias de prevenção, monitoramento e tratamento da obesidade infantil.
Abstract Childhood obesity is associated with a state of chronic low-grade inflammation characterized by dysregulation of both innate and adaptive immunity. Structural alterations in adipose tissue, including adipocyte hypertrophy, immune cell infiltration, and fibrosis, are also associated with insulin resistance, endothelial dysfunction, and an unfavorable metabolic profile. Within the adaptive immune system, alterations have been described in several lymphocyte populations, including an imbalance between Th17 and regulatory T (Treg) cells, changes in B-cell populations, and associations between these immune alterations and parameters such as BMI, HOMA-IR, glucose metabolism, and lipid profile, highlighting the contribution of adaptive immune responses to the pathophysiology of pediatric obesity. In addition, alterations in NK cells and immune cell energy metabolism suggest an early immunometabolic reprogramming associated with excess adiposity. Collectively, the available evidence indicates that obesity during childhood and adolescence is characterized by persistent immune activation accompanied by metabolic disturbances. A better understanding of these interactions may contribute to the development of novel strategies for the prevention, monitoring, and treatment of childhood obesity.
