Avaliação da fotobiomodulação na absorção do anestésico tópico e redução da dor durante a anestesia infiltrativa local na clínica odontológica

Resumo

O objetivo desse trabalho foi avaliar a aplicabilidade da fotobiomodulação (FTB) para auxiliar na absorção do anestésico tópico e na redução da dor durante a anestesia infiltrativa local na clínica odontológica. Foi realizado um estudo de boca dividida em 20 pacientes que necessitaram de anestesia infiltrativa. Os dentes que necessitaram de anestesia infiltrativa do mesmo paciente foram divididos aleatoriamente em 2 grupos (n =20): Sham (S) e Fotobiomodulação (FTB). No grupo FTB, foi feita a fotobiomodulação sistêmica e local. A fotobiomodulação sistêmica foi feita com laser de baixa intensidade com energia de 60 J em modo contínuo por 10 minutos na artéria radial, potência de 100 mW e comprimento de onda 660 nm (luz vermelha). A fotobiomodulação local foi feita com energia 6 J por 1 minuto , na região da mucosa oral, potência de 100 mW e com o comprimento de onda de 780 nm (luz infravermelha). Após a FTB, foi feita a aplicação do anestésico tópico benzocaína 20%. No grupo Sham, foi feita a simulação do laser na artéria radial e na mucosa oral do paciente previamente a aplicação do anestésico tópico benzocaína 20%. A perceção da dor em ambos os grupos foi avaliada utilizando a Escala Visual Analógica (EVA). Os resultados foram submetidos ao teste t. Houve redução significativa da dor durante a anestesia infiltrativa nos dentes que receberam a fotobiomodulação antes da aplicação do anestésico tópico em comparação com os dentes que foram anestesiados sem a aplicação do laser (p<0.0001). Pode-se concluir que a utilização da fotobiomodulação antes da aplicação do anestésico tópico pode ser uma alternativa clínica para controle de dor durante anestesia infiltrativa.
Abstract This work aims to evaluate the applicability of the photobiomodulation (PMB) to help in the absorption of the topical anaesthetic, and in the reduction of the pain during the local infiltrating anesthesia in the dental clinic. A split-mouth study was made in 20 patients who needed previous infiltrating anesthesia. The teeth that required infiltrating anesthesia, from the same patient, were randomly divided into 2 groups (n =20): Sham (S), and Photobiomodulation (PMB). In the PMB group, was performed the local and systemic photobiomodulation. The systemic photobiomodulation was performed with a low-intensity laser, with power of 60 J in continuous mode, for 10 minutes, in the radial artery, power of 100 mW in wavelength 660 nm (red light). The local photobiomodulation was made with 6 J for one minute, in the oral mucosa region, power of 100 mW, and wavelength of 780 nm (infrared light). After the PMB, the topical anesthetic 20% benzocaine was administered. In the Sham group, it was used the simulation of the laser on the patient's radial artery and oral mucosa prior to the application of the topical anesthetic 20% benzocaine. In both groups, the perception of pain was measured using the Visual Analogue Scale (VAS). The results were submitted to the t test. There was a significant reduction in pain during the infiltrating anesthesia in the teeth that received photobiomodulation before the application of the topical anesthetic compared to the teeth that were anesthetized without the laser application (p<0.0001). It can be concluded that the use of the photobiomodulation before the application of the topical anesthetic can be a clinical option in pain control during infiltrating anesthesia.
